Information for the Injured Worker

As an injured worker, you have both benefits and responsibilities if you are hurt on the job and your employer is insured by Chesapeake Employers. It is important for you to become familiar with all of the benefits Chesapeake Employers provides. By utilizing our claim services, you are taking advantage of an integrated system of benefits that will provide you with quality medical care so that you can recover from your injury quickly and return to work as soon as medically possible.

Caring and prompt claims services to help injured employees:

  • Toll-free customer service number: 1-800-264-4943;

  • Pre-authorized prescriptions with no out-of-pocket costs;
    (This is a free first prescription fill for injured workers. It is available after you have reported your injury to your employer and your employer has notified/reported the injury to Chesapeake Employers.)

  • Durable medical equipment covered;

  • Vocational rehabilitation services;

  • Prompt payment for lost wages; and

  • Prompt payment of medical bills.

Important reminder after the injury has been reported to Chesapeake Employers:

You must stay in contact with your employer and your Chesapeake Employers' Claims Adjuster. Let he or she know how your care and treatment are progressing. You must also actively participate in recommended medical treatment plans. The ultimate goal of your medical treatment is for a speedy recovery and a healthy return to work.

  • Claimant Authorization Agreement for Direct Payments PDF

    Claimants who would like to have their workers' compensation benefit checks directly deposited to their personal bank accounts must complete this form. Please note: Chesapeake Employers can only pay Permanent Total benefits, Fatality benefits and, in some cases, Serious Disability benefits biweekly by direct deposit on request. In addition, the payout must be for 6 months or more of future biweekly benefits. The weekly benefit for temporary total disability is NOT available for direct deposit. Please consult your Chesapeake Employers' Claim Adjuster for more information.

  • Medical Travel Expense Form PDF

    Claimants are entitled to reimbursement for travel expenses for medical treatment resulting from a work injury. This completed form along with appropriate receipts are required for reimbursement.
